MyMichigan Health has opened its newest pharmacy-driven anticoagulation clinic at MyMichigan Medical Center Sault, offering the community a vital resource, and ensuring that patients receive comprehensive, high-quality care. It is the tenth of its kind in the MyMichigan system. This innovative clinic is set to transform chronic care management for patients on anticoagulation therapy, significantly improving health outcomes and enhancing patient satisfaction.

For more than 30 years, MyMichigan pharmacists have been at the forefront of safe anticoagulation monitoring across the state of Michigan. Patients on anticoagulation therapy, such as those taking warfarin (Coumadin), face complex risks that necessitate careful and frequent monitoring. The newly opened clinic in Sault is equipped with a pharmacist-led model that allows for regular face-to-face interactions, immediate dose adjustments, and comprehensive education on anticoagulation therapy.

Previously, patients in the Sault area seeking anticoagulation management had to travel to Newberry to access such specialized care. With the opening of this new clinic, they now have local access to personalized management and education, tailored to their specific needs. This development is not only convenient but is also expected to lead to better patient outcomes and satisfaction.

“Our pharmacist-led model ensures that patients receive timely monitoring, prompt dose adjustments, and clear, accessible guidance,” said Miranda Maitland, PharmD, B.C.P.S., F.M.P.A., pharmacist and clinical coordinator at MyMichigan Medical Center Sault. “With pharmacists at the patient’s side, individuals benefit from managing their anticoagulation safely, confidently, and with greater peace of mind.”

Patients currently on warfarin (Coumadin) who are interested in being managed by the Anticoagulation Clinic in the Sault are encouraged to contact their primary care provider to set up a referral. Those interested in more information on the Anticoagulation Clinic may call (906) 635-4647.
