Breast Cancer Survivorship Care Planning

For some survivorship starts when one is first diagnosed with breast cancer and continues for the rest of a woman’s life. For others, survivorship may begin after their initial treatments which may have included surgery, radiation therapy, chemotherapy and immunotherapy have been completed. Also, depending on one’s type of cancer, long-term treatment such as hormonal therapy may become of one’s cancer journey.
The important thing to remember isn’t when you categorize when your survivorship begins, it is important to look at one’s long term health and wellness.
A Survivorship Care Plan is a coordinated, post-treatment plan for the cancer survivor, oncology team, primary care physician and other healthcare providers. The plan has been found to improve the quality of care of survivors as they move beyond their cancer treatment.
