Description

Free Advance Care Planning Community Facilitator Training

The Advance Care Planning Community Facilitator training is open to health care professionals and offers continuing education credits for Community Health Workers, Registered Nurses, and Social Workers. Participants will complete pre-work assignments and role plays to receive credit.

In this training, participants will learn how to educate adults on the importance of creating a legal document that names their patient advocate, how to explore various treatment preferences and assist adults with documenting these treatment preferences, and how to assure that this legal document is appropriately witnessed. Participants will also receive education on landmark cases, communication techniques, and medical orders related to advance care planning.

Virtual participants are required to use the audio and video functions of Microsoft Teams to complete the roles plays required for continuing education credits.
