MyMichigan Health’s Community Education Department is offering a chronic disease self-management program, a workshop for adults with at least one chronic health condition. The interactive program focuses on disease management skills including decision-making, problem solving and action planning, and aims to increase confidence, physical and psychological well-being, knowledge of ways to manage chronic conditions and motivation to manage challenges associated with chronic diseases. The six-week series will be led by two trained leaders, and includes a workbook that participants will receive and use during the program. Caregivers are welcome to join the class.
Workshop topics include:
- Techniques to deal with symptoms of chronic conditions, such as fatigue, pain, sleeplessness, shortness of breath, stress and emotional problems
- Maintaining and improving strength and endurance
- Fall prevention
- Healthy eating
- Better breathing techniques
- Medication management and working effectively with health care providers
- Communication with friends and family, with oneself and with the health care system
