Hospice Services

Hospice care is designed to meet the unique needs of each patient and their family—physically, emotionally, and spiritually. At MyMichigan Hospice, our team works together to provide compassionate, personalized care and support.

Care Team & Support Services

  • Medical Director &Nurse Practitioners
    • Oversee patient care and work closely with your primary care provider
  • Hospice Registered Nurses
    • Available 24 hours per day/7 days per week for care and support
  • Home Health Aide
    • Assist with personal care including, hygiene, grooming and nutrition
  • Social Workers
    • Provide emotional support and help connect patients and families with community resources
  • Therapy Services
    • Physical, Occupational, and Speech Therapy may be available to support safe movement, communication and daily activities available for evaluation for safe mobility and eating.
  • Spiritual Support
    • Offered based on the patient's and family's beliefs and preferences

Additional Support Services

  • Music Therapy
    • Uses music to support well-being and comfort. Benefits may include:
      • Expressing feelings and memories
      • Reducing pain stress and anxiety
      • Improving mood and relaxation
      • Decreasing feelings of loneliness
      • Supporting communication and moments of clarity
  • Volunteers
    • Provide companionship, respite care, pet therapy and other support
  • Medications and Supplies
    • Provided as needed for comfort, safety and care

Family & Bereavement Support

  • Bereavement Services
    • Support is available during illness and continues for up to 13 months after a loved one's passing
  • Grief & Loss Support Groups
    • Offered in select communities to help individuals cope with loss.

Woodland Hospice House

The Woodland Hospice House is available for respite and long term stays, based on availability. The peaceful setting provides comfort, privacy, and personalized care for patients and their families.
