Meet Our Hospice Team

MyMichigan Hospice has a designated staff led by a medical director and nurse practitioners who specialize in end of life. The team meets regularly to review the patient's and family's condition and provide comprehensive recommendations for care.

The hospice care team consists of:

• Medical Director and Nurse Practitioners - specialize in symptom management to oversee patient care in conjunction with the patient’s primary care provider. A hospice provider is on call, 24 hours a day/7 days a week, to consult with the hospice team on patient needs. 
• Hospice Registered Nurses - dedicated to the management of physical and emotional support. They educate patients and caregivers of the disease process and comfort measures available. (24 hour access)
• Social Workers - available to provide counseling, education, community resource linkage and assistance with end-of-life care planning.
• Home Health Aides - assist in bathing, grooming and nutrition according to the patient's plan of care.
• Rehabilitation Therapists - Physical, occupational and speech therapists may be a part of the care team, they assist with safe mobility and assistive devices.
• Volunteers - often provide well-deserved rest for the patient's family and companionship for the patient.
