Meet Our Hospice Team

At MyMichigan Hospice, our care team is led by a Medical Director and Nurse Practitioners who specialize in end-of-life care. The team meets regularly to review each patient’s condition and provide comprehensive care recommendations for care.

Our Hospice Care Team Includes

  • Medical Director and Nurse Practitioners
    • specialize in symptom management and oversee patient care along side your primary care provider. A hospice provider is on call, 24/7 to support he team and consult on patient needs. 
  • Hospice Registered Nurses
    • Provide physical and emotional support, educate patients and caregivers on the disease process, and explain available comfort measures. Available 24/7.
  • Social Workers
    • Offer counseling, education, assistance with end-of-life care planning, and connection to community resources.
  • Home Health Aides
    • Help with bathing, grooming, and nutrition according to the patient's individualized care plan.
  • Rehabilitation Therapists
    • Physical, Occupational, and Speech therapists may assist with safe mobility and use of assistive devices.
  • Spiritual Support
    • Available as defined by the patient's and family's wishes.
  • Volunteers
    • Provide companionship for patients and respite for family caregivers.
