Integrated Behavioral Health Program
Our Integrated Behavioral Health Program helps make mental health care easier to access by offering services right within the primary care office. This team-based approach includes a Behavioral Health Therapist, and often a Clinical Social Worker or Licensed Professional Counselor, who works closely with your medical care team.
Therapists partner with your provider to support your overall health. They may meet with you during a regular office visit, call to help start care, schedule an in-person or virtual therapy appointment, or respond to a mental health crisis when needed.
What is Integrated Behavioral Health
Is a team approach including a Behavioral Health Therapist and often times a Clinical Social Worker or Licensed Professional Counselor located in the primary care office or specialty offices who work hand-and-hand and partner with your medical care team. The therapist can introduce therapy during an office visit, make a phone call to you to establish care, set-up in-office or virtual therapy sessions or respond to a crisis.
By actively addressing multiple aspects of your health within the primary care office, patient care can be less fragmented and more focused on addressing your “total health care needs.”
Therapists Can Assist With
- Providing short-term evidence-based therapy such as cognitive behavioral therapy, problem solving therapy, interpersonal counseling, behavioral activation and motivational interviewing.
- Conducting a review of your behavioral health information.
- Providing education about your specific behavioral health concerns.
- Helping you understand medications prescribed by your provider for behavioral health concerns.
- Working with you to create personal goals and action plans to make changes to improve emotional well-being.
- Connecting you to community resources for behavioral health support.
What Services Can You Expect
- Immediate interventions to assist with patients attending provider appointments who are experiencing a crisis.
- Behavioral Health Therapy starts with an initial assessment and development of a Care Plan or referral for more in-depth evaluation or specialist, if indicated.
- Follow-up appointments to provide short-term therapy utilizing cognitive behavioral therapy, motivational interviewing, problem solving therapy, behavioral activation, interpersonal counseling for primary care setting, as well as a focus on mindfulness.

Collaborative Care Program

MyMichigan Health has a team of behavioral health specialists working right with our primary care offices to make mental health care easier to access. These experts work closely with your providers to offer support and treatment for conditions such as depression and anxiety.
What is Collaborative Care
Collaborative Care is an evidence-base approach that brings together a team of professionals to support you mental health needs. Communication is key.
Your Care Team Includes
- Primary Care Provider (PCP): Your provider will oversee your overall health and stays informed of your mental health progress
- Behavioral Health Therapist: Your therapist will work closely with you and your PCP to implement treatment plans.
- Psychiatric Consultant: A team of psychiatrists may be consulted to provide guidance to your PCP and therapist, if treatments need adjustment.
How It Works
- Your PCP and therapist communicate regularly to track your progress.
- The psychiatric consult meets with the your therapist to discuss treatment options and your progress.
- With your permission, a psychiatric consult may meet with you in person or via telemedicine for additional support.

Several primary care offices participate in the Integrated Behavioral Health & Collaborative Care Programs as part of the primary care team for ongoing treatment. If your primary care office doesn’t have a therapist, please ask for a referral.
