At MyMichigan Health, we believe that care doesn’t end when cancer treatment does. Survivorship is an important part of the cancer journey, and we are committed to helping our patients thrive in the years that follow.

Following a recommendation by the Institute of Medicine in 2006, all cancer survivors should receive a personalized Survivorship Care Plan—a coordinated roadmap developed in collaboration with the oncology care team, primary care provider, and other health professionals. These plans are designed to help survivors understand their medical history, anticipate their needs, and take an active role in managing their long-term health.

What Is a Survivorship Care Plan?

A Survivorship Care Plan is provided after completion of primary cancer treatments and includes:

  • Contact information for the survivor’s care team
  • Summary of the cancer diagnosis and treatments received
  • Recommended schedule for follow-up tests and screenings
  • Guidance on managing side effects or long-term symptoms
  • Information about what to expect in recovery and beyond

These plans empower survivors to monitor their health, manage any ongoing effects of treatment, and ensure continuity of care between specialists and primary providers.

Who Creates the Plan?

Survivorship Care Plans are developed by a member of your care team, such as an oncology nurse, nurse navigator, oncologist, nurse practitioner, or physician assistant. The plan is created in collaboration with the patient to reflect their individual treatment journey and ongoing health goals.
