Biography
Colin McGraw, P.A.-C. specializes in acute care. He sees patients at MyMichigan’s Continuing Care Clinic in Midland.
The role of the Continuing Care Clinic is to support patients during transitions in care, such as after discharge from a hospital, Emergency Department or long-term care facility, or while they are establishing with a new primary care provider. Colin guides patients through their transition or recovery plan and bridges the gap until their primary provider can resume their care. This may include reviewing their medications, diagnoses or discharge instructions; making referrals for follow up care; or helping patients troubleshoot issues such as transportation, mobility or tasks of daily living.
Philosophy of Care
“I have always enjoyed helping people, which is why I joined the medical field,” Colin said. “I feel that the Continuing Care Clinic provides a unique opportunity to help those in need in the community.”
“My philosophy of care: Care begins with listening. I would like to build a trusting relationship with my patients, so they feel comfortable and reassured in the care that they are receiving. The most satisfying aspect of my work is having the opportunity to help improve others’ wellbeing.”
During his free time, Colin enjoys spending time with family, including his wife and son, staying active outdoors, hunting, playing golf, and playing yardgames.
