Eligibility and How to Apply
Application Requirements
- A Doctor of Pharmacy (Pharm.D.) degree from a college of pharmacy accredited by the Accreditation Council for Pharmacy Education
- A pharmacist license or be eligible for licensure in the state of Michigan
- Interested applicants must submit the standard application materials through PhORCAS by the deadline of January 2, which includes:
- Letter of Intent
- Recommendations from three professional sources
- Copy of your College of Pharmacy transcripts
- Copy of your Curriculum Vitae
- Participation in the ASHP Residency Matching Program
- MyMichigan Health Midland ASHP Match #288213
This residency agrees that no person at this residency will solicit, accept, or use any ranking-related information from any residency applicant.
Additional Requirements
Pharmacy Residents must meet all hiring policies set by MyMichigan Health including but not limited to:
- Immunization
- Physical Health Review
- Drug Screening
- HIPAA training, safety training, and any other training deemed necessary by MyMichigan Health
